Overview
We're looking for a 7-on. 7-off. On-Call RN Full Time

You will act as primary liaison between physician, patient/family and hospice team.

Provide direct patient care services to hospice patients and supportive care to the patient's family on an on-call basis
-
Manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care
-
Evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient
-
Maintain all assigned patient clinical records
-
Participate in company quality assessment and performance improvement programs
About You
Qualifications - What you'll bring
-
Current RN license in the state of requested employment (or eligibility to obtain)

1+ years of nursing experience
-
A hospice heart: compassionate, calm, and dependable in urgent care situations
-
Excellent clinical judgment and the ability to provide support in emotionally intense settings
-
Comfortable providing care in patients' homes, facilities, or wherever they reside
-
Valid driver's license, automobile insurance, and reliable transportation
-
Current CPR certification
Preferred Experience (Not required):
-
1+ years of hospice, home health, oncology, or palliative care experience
-
Familiarity with symptom management, triage, and after-hours home-based care
-
Prior experience in on-call or overnight nursing roles
-
Strong documentation and electronic medical records (EMR) skills
