Position Summary:
Facilitates and coordinates the process for all acute transfers in to the organization in accordance with the clinical standards and guidelines. Manages all calls related to patient transfers to Trinity Health Grand Rapids (THGR) from acute care facilities, physician offices and long term care seeking to have patient admitted to THGR or evaluated in the Emergency or OB department.
Minimum qualifications:

Bachelors Degree in Nursing or other health related field is required

Current Michigan license as a Registered Nurse

Active BLS certification
-
Current knowledge of patient care practices and health care systems

Five years of current and progressive clinical and leadership experience in an acute care setting, including involvement in direct patient care and educating staff
What the RN Coordinator-Admission Line will do:
-
Answers and responds to all incoming calls for purposes of patient transfer for admission or evaluation at Trinity Health Grand Rapids

Serves as a resource for sending facilities and THGR attending/admitting physicians.

Provides direction and assistance with identifying correct admitting service needed for patient transfer and/or evaluation.

Facilitates ED to ED communication, Physician to Physician conversation for transfers and admissions.

Collaborates with Clinical Resource Coordinator to plan for placement of patient.

Facilitates receiving admitting orders from admitting physician if needed.
