The Public/Community Health Nurse (PHN) is responsible for the mental and physical well-being of the community by ensuring the public's health through health promotion, disease prevention, treatment, and education. The Public/Community Health Nurse (PHN) can work in a variety of institutions such as the community, hospitals, clinics, or long-term care facilities while collaborating with state regulators, doctors, and other healthcare professionals to keep the community safe and healthy.
Minimum Requirements:

Active Nursing Licensure in the state practicing

Two (2) years of public health or community experience preferred

Complies with all relevant professional standards of practice

Current CPR if applicable

TB questionnaire, PPD or chest x-ray if applicable

Current Health certificate (per contract or state regulation)

Must meet all federal, state and local requirements

Must be at least 18 years of age
The Public Health Nurse II serves as a certified Long-Term Services & Supports (LTSS) Screener, conducting home visits, assessing eligibility for Medicaid-funded long-term care programs, and providing public health nursing services. The role blends clinical assessment, case management, care coordination, and public health outreach, with a strong emphasis on person-centered practice, cultural agility, and independent decision-making.
Key Responsibilities
LTSS Screening & Clinical Assessment

Conducts LTSS screenings with multidisciplinary teams.

Performs home visits to determine health, safety, and level-of-care needs per DMAS guidelines.

Assesses risks for medically fragile, disabled, or at-risk adults and children.
-
Uses the Uniform Assessment Instrument (UAI) for comprehensive evaluations.
Case Management & Care Coordination

Develops care plans and coordinates services for older adults, adults with disabilities, and children.

Makes referrals and recommendations based on client needs.

Provides crisis intervention when necessary.
Public Health Nursing Functions

Conducts outreach, case finding, health promotion, and education.

Implements public health interventions and provides consultation.

Maintains nursing competencies through required training.
Collaboration & Leadership

Works independently and collaboratively with diverse clients and teams.

Mentors new staff and provides training to colleagues and community groups.

Participates in interdisciplinary team (IDT) meetings.
