POSITION SUMMARY:
This position has individual responsibility for abstracting data as defined by the various measure sets/indicators. This data may be entered into computer database and/or Microsoft Word, Excel, or manually entered on a form. Monitor for compliance to maintain hospital standards of care in accordance with TJC, state, and federal regulations. As Disease Specific Care (DSC) Coordinator this role will provide education and ACS support to the Emergency Department, Cath Lab, Critical Care, telemetry, CPRU and all areas within the cardiac services line. As DSC Coordinator, this position will support the ongoing compliance for TJC certification.
ESSENTIAL FUNCTIONS:

Drives current DSC certification through performance improvement activities and standards compliance.

Systematically abstracts specified data that is complete, accurate, and consistent.

Concurrently reviews all required patient records for compliance with core measures and disease specific indicators.

Demonstrates and facilitates compliance with TJC, CMS, AHCA requirements regarding timeliness of submission of data

Reports and present accurate quality information both internally and externally

Exhibits responsibility and accountability for assigned work, works independently

Uses various software programs to develop, generate and update documentation

Understands all functional areas of the hospital.

Facilitates the Disease Specific Care Committee

Assist in reviewing and recommending protocols for best practice treatment across continuum of care.

Successfully integrates the DSC programs into hospital operations through support of an interdisciplinary approach to education, care and current treatment guidelines.

Assists in tracking the quality outcomes of emergent and non-emergent DSC patient to continually improve throughout facility and facilitates changes to improve metrics from first point of contact to final disposition.

Works collaboratively with Emergency Medical Services to assure quality outcomes and optimum care for our DSC patients - from pre-hospital with EMS and emergent care to treatment and disposition.

Assist in education of nursing, medical and ancillary staff on best practices for the DSC patients based on recommended guidelines through a nationally recognized clearinghouse.

Coordinates and provides community education and attends health fairs based on community needs.

EDUCATION REQUIRED AND/OR PREFERRED:
· AS degree in Nursing
LICENSURE/CERTIFICATION:
· Current State of Florida license for Registered Nurse or appropriate compact licensure. If compact license held, active FL RN license required within 90 days of hire.
· Abstracting experience preferred
EXPERIENCE REQUIRED OR PREFERRED:
· 3-5 years clinical experience
