POSITION SUMMARY:
Wood River Health provides team-based care consistent with the principles and standards of a Patient Centered Medical Home (PCMH).
The Registered Nurse provides support to the provider with direct patient care activities such as assisting with procedures, performing point of care tests, and obtaining data using equipment i.e., EKG. The Functions as Team RN are to oversee, support, and delegate care given to patients. Promotes health through education, collaboration, and outreach activities. Utilizes nursing process, provides physiological and psychological support to patients. Participates in professional development by attending programs, workshops, in-services both internal and external, as well as department meetings.
ESSENTIAL DUTIES; include but are not limited to:
- Knowledge and Navigation experience with EHR; using the 4-point check, preferred.
- Must be able to prioritize and complete information requested in a timely fashion.
- Medication administration.
- Preparation of Vaccine and administration, required.
- Knowledge of pediatric immunizations preferred.
- Communicate with the patient's provider on an ongoing basis.
- Secure guidance or help as needed with department manager and support staff.
- Educate patients regarding treatment procedures, medications, or physicians' instructions to patients.
- Telephone and walk-in triage of patients.
- In collaboration with the Finance Department, conduct 340b Drug Pricing Program self-audits to ensure program integrity and compliance in the following areas: provider eligibility, site eligibility, patient eligibility, payer status, scope of service and inventory management.
- Participate in professional development by attending programs workshops, in-services, assigned committees, department and staff meetings.

- Operate within the scope of the Health Information Portability and Accountability Act to safeguard the privacy of protected patient health information

SUPERVISORY RESPONSIBILITIES:
None
EDUCATION AND EXPERIENCE:
- Graduate of accredited nursing program, BSN preferred
- One to two years related experience, preferred
- Electronic Medical Record experience, preferred
- Experience working in a community health center, preferred
- Ability to effectively use MS Office Suite, Internet, and email
- Cultural sensitivity necessary to work with a diverse patient and staff population
LICENSURE AND CERTIFICATION:
- Full, unrestricted, Registered Nurse license in the State of Rhode Island
- Additional and ongoing care management training and certification is expected as part of licensure maintenance
- CPR Certification, required
PHYSICAL DEMANDS AND WORK ENVIRONMENT

While performing the duties of this job, the employee is regularly required to lift or carry 10 pounds or less, and 11-20 pounds occasionally. For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
