We are seeking an experienced Travel Home Health Registered Nurse (RN) to provide high-quality patient care in a home health setting throughout the Huntington, West Virginia service area. The ideal candidate will have prior home health experience, be comfortable working independently in the field, and possess strong clinical assessment and documentation skills.
Location: Huntington, WV 25705
Coverage Area: Cabell, Putnam, and Wayne Counties
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
On-Call Requirement: One week of on-call coverage every 4-6 weeks
Contract Type: Travel/Temporary Assignment
Mileage Reimbursement: Yes, between patient visits
Pay Rate: $ 55.92/hr.
Responsibilities

Conduct comprehensive patient assessments in the home setting

Develop, implement, and manage individualized patient care plans

Provide skilled nursing care and interventions based on physician orders

Monitor patient progress and communicate changes in condition to the care team

Educate patients and caregivers on disease management, medications, and treatment plans

Coordinate care with physicians, therapists, and other healthcare professionals

Complete accurate and timely documentation in the electronic medical record

Maintain compliance with all home health regulations and agency standards
Required Qualifications

Active Registered Nurse (RN) license

Previous Home Health nursing experience

Experience using Homecare Homebase (HCHB)

Valid driver's license and reliable transportation

Strong clinical assessment and communication skills

Ability to work independently in a field-based environment
