The Clinical Team Manager (RN) is the clinical leader responsible for supporting and guiding our field clinicians while ensuring patients receive safe, high-quality, coordinated care in the place they call home.

As a Clinical Team Manager (RN) , you'll serve as the bridge between patient care and operational excellence-leading clinicians, supporting quality outcomes, coordinating care delivery, and helping your team succeed every day.

As a Clinical Team Manager, you will:

Lead and support a multidisciplinary team of nurses, therapists, and clinicians.

Oversee patient care, assignments, referrals, admissions, and care transitions.

Review care plans, physician orders, clinical documentation, and OASIS assessments.

Drive quality, compliance, patient outcomes, and operational performance.

Coach, mentor, train, and engage field clinicians.

Support recruiting, onboarding, team meetings, and staff development.

Provide patient visits and field support when needed.

Build a culture of collaboration, accountability, and clinical excellence.

What You'll Need:

Active, unrestricted RN license and an associate nursing degree; BSN preferred.

At least two years of RN experience in home health or hospice.

Leadership experience and knowledge of OASIS and home health regulations is required

Strong clinical judgment, communication, coaching, and technology skills.

Willingness to travel throughout the territory
-
valid driver's license, reliable transportation, and auto insurance.

Ability to lift, push, pull, and support up to 50 pounds.
