Adecco Healthcare & Life Sciences is seeking an experienced Licensed Practical Nurse (LPN) for a Home Health contract in Peoria, AZ . This is a 13-week assignment providing skilled nursing care to patients in their homes throughout the Phoenix West Valley service area.
Job Details
· Coverage Area: Phoenix, Glendale, Peoria, Goodyear, Litchfield, Litchfield Park, Surprise, Tolleson, Buckeye, Youngtown, Sun City, Sun City West, and Anthem
· Schedule: Monday-Friday, 8:00 AM - 4:30 PM
· Pay: $34 to $36/hr, Mileage reimbursed between patient visits
Responsibilities
· Provide skilled nursing care to patients in their homes throughout the assigned service area
· Complete scheduled patient visits and provide care based on individual patient needs and care plans
· Provide wound care, compression wraps, and wound VAC management
· Perform catheter changes and provide catheter care
· Provide tracheostomy care
· Provide NG tube and nephrostomy tube care
· Perform urinalysis (UA) as needed
· Provide disease education and patient/family education
· Monitor patient conditions and report changes to the appropriate healthcare team
· Complete accurate and timely clinical documentation
· Travel between patient visits throughout the assigned coverage area
Requirements
· Current Arizona LPN license
· Arizona Level I Fingerprint Clearance Card
· Current CPR certification
· Ability to work independently in a field-based environment
· Reliable transportation for travel between patient visits
· Strong communication, organization, and documentation skills

Pay Details: $34.00 to $36.00 per hour
