Adecco Healthcare & Life Sciences is seeking an LVN Intake Coordinator to join our team in Clearlake, CA 95422 . We are looking for a detail-oriented professional LVN who can seamlessly manage the transition of care and keep our clinical operations running smoothly.
The Impact You'll Make
As an Intake Coordinator, you are the central hub for patient care coordination. You'll be the primary point of contact for physicians, families, and healthcare teams, ensuring every referral is handled with urgency and accuracy.
Core Responsibilities

Manage Referrals: Coordinate incoming office referrals and verify insurance coverage immediately.

Expert Communication: Act as the liaison between nursing, therapy, social services, and physicians regarding orders and schedule changes.

Patient Advocacy: Follow up on hospitalized patients to ensure a smooth transition back to agency services.

Lead the Flow: Apply your healthcare expertise to meet scheduling goals and serve as a resource for the workgroup.
Pay & Application
Requirements:

License: Active LVN (Licensed Vocational Nurse) or RN licensure in the state of practice.

Experience: 3+ years of relevant healthcare experience preferred.

Education: High School Diploma/GED required (Associate's or Technical degree preferred).

Compliance: Ability to meet vaccination requirements (medical/religious exemptions may apply).

Pay Details: $46.00 per hour
