Adecco Healthcare & Life Sciences is seeking an experienced RN Case Manager to support a leading healthcare organization. This is an excellent opportunity for a licensed nursing professional who thrives in a fast-paced hospital environment and is passionate about care coordination, discharge planning, utilization review, and improving patient outcomes.
In this role, you will collaborate with interdisciplinary teams to ensure patients receive the appropriate level of care while supporting efficient transitions throughout the continuum of care. The Case Manager serves as a key resource for physicians, nurses, social workers, patients, families, and third-party payers to drive quality outcomes and appropriate resource utilization.
Schedule Requirement: Must be available to work every other weekend.
Key Responsibilities

Coordinate patient care transitions, discharge planning, and follow-up services

Perform utilization review for appropriateness of admission, level of care, and length of stay

Collaborate with physicians, nurses, social workers, and interdisciplinary teams to develop and implement patient care plans

Communicate with third-party payers and review organizations to support reimbursement and authorization processes

Identify opportunities for quality improvement, resource optimization, and positive patient outcomes

Serve as an educational resource regarding case management processes and regulatory requirements

Maintain accurate documentation, reporting, and compliance records
Pay
The anticipated wage for this position is between $78.00 and $101.00 per hour. Hourly wage depends upon experience, education, geographic location, and other factors.
Qualifications

Bachelor's Degree from an accredited college or university

Active RN license required

Strong knowledge of discharge planning, utilization review, case management models, and healthcare regulations

Experience coordinating care across the continuum in a hospital setting

Ability to interpret clinical information, analyze data, and support quality improvement initiatives

Strong communication, collaboration, and conflict resolution skills

Knowledge of reimbursement processes, hospital operations, and patient care coordination
