Position Purpose
In the Emergency Department (ED) at Saint Francis Hospital and Medical Center , the Nurse Senior - Navigator, informally known as the Complex Care Coordinator, plays a crucial role in supporting emergent and high-risk patients with complex medical, social, and/or behavioral health needs. The Registered Nurse (RN) in this role collaborates closely with the ED care team to deliver coordinated, patient-centered care and support safe, effective transitions across the continuum.
What You Will Do

Conduct comprehensive, real-time assessments of patients' medical, psychosocial, and behavioral health needs upon presentation to the Emergency Department.

Collaborate closely with Emergency Department physicians, advanced practice providers, nursing staff, case management, and ancillary teams to support individualized care planning for complex and high-risk patients.

Coordinate referrals and facilitate connections to appropriate inpatient services, specialty providers, community resources, and support programs to promote safe dispositions and effective transitions of care.
Minimum Qualifications
Required:

Graduation from an accredited nursing program, with a Bachelor of Science in Nursing (BSN) strongly preferred and generally expected for successful candidates.

Valid RN license in Connecticut.

The ideal candidate will have a minimum of five (5) years of Emergency Department and/or acute inpatient nursing experience, with demonstrated ability to manage complex, high-risk patient populations.

Compensation: Salary range $86,304 - $133,328, based on a 40-hour full-time equivalent; compensation will be prorated based on a 24-hour workweek
