Hospice RN Case Manager
Location: Coverage area spans Brookline/Brighton to Mansfield
(Scheduler clusters visits by geographic area)

Start Date: July 13
This assignment is to cover a 6-month maternity leave. The RN must be available to start a few weeks earlier if the incumbent nurse begins leave sooner.

As a Hospice RN Case Manager, you'll play a vital role in supporting patients and families with comfort-focused, holistic care during the end-of-life journey.
Key Responsibilities

Conduct 5-6 home or facility visits per day

Assess patient condition and monitor symptom progression

Collaborate with physicians and interdisciplinary team members on individualized care plans

Provide comfort-focused care tailored to each patient's needs

Partner closely with caregivers to ensure continuity of care

Administer medications and manage pain and symptom control

Respond to acute changes in condition and escalate to physicians as needed

Document care accurately and thoroughly

Maintain high ethical standards and adhere to safety protocols

Demonstrate strong clinical judgment, attention to detail, and compassionate communication
Working Conditions

Care provided in private homes, assisted living facilities, and nursing homes

Travel required throughout the designated coverage area

Remote work as needed for documentation

Valid driver's license required
Member-Facing Role: Yes - in-person patient interaction required
Requirements

BSN with an active, unrestricted Massachusetts RN license

Minimum of 2 years of hospice case management experience
Preferred Qualifications
- Experience with electronic medical records (EMR) systems
