Your Key Responsibilities
Clinical Assessment & Care Planning

Holistic Evaluations: Conduct thorough medical assessments for incoming individuals, partnering closely with the Intake team to ensure appropriate community placement.

Care Plan Leadership: Collaborate with physicians to develop, implement, and routinely update individualized service and nursing care plans that prioritize long-term health and independence.

Protocol Development: Generate robust healthcare protocols for specific medical conditions outlined in individual health and care plans.
Medical Administration & Coordination

Safe Administration: Manage and administer medications in strict accordance with clinical guidelines, ensuring meticulous documentation and compliant record reviews.

High-Risk Management: Oversee the clinical care of high-risk medical conditions and coordinate private duty nursing services as needed.

Physician Liaison: Maintain seamless communication with primary care physicians and specialists, accompanying individuals on medical visits when required.
Training, Mentorship & Collaboration

Empowering Others: Design tailored medical training tools and deliver clinical skills training to clients, staff, mentors, respite providers, and biological families.

Onboarding Excellence: Present vital medical skills components during Pre-service Orientation for new team members.

Interdisciplinary Leadership: Actively contribute to Individual Health Plan (IHP) teams, Human Rights Committees, and Health & Safety administrative meetings.

Community Support: Participate in an on-call system and assist with essential transitional or household coordination-such as safe transportation to appointments-to guarantee continuity of care.
What You Bring to the Team
Education & Experience
Certificates, Licenses, and Registrations

Current and valid State RN License in good standing.

Current CPR / First Aid certification.

Valid driver's license, active car registration, and auto insurance (required for providing participant transportation).
Physical Requirements & Safety

Physical Strength: Heavy Work. This position requires exerting up to 100 pounds of force occasionally, up to 50 pounds frequently, and/or up to 20 pounds constantly to safely move objects and support individual care.

Bloodborne Pathogens: This role involves tasks that carry a potential exposure to blood, body fluids, or tissues; standard clinical precautions and training are an expectation of employment.

Travel: Ability to travel locally as needed to support community-based services.
