We are seeking an experienced RN Case Manager for a contract opportunity in Presque Isle, Maine. This role is responsible for conducting comprehensive case management assessments, coordinating patient care transitions, supporting discharge planning efforts, and ensuring compliance with CMS regulations and hospital policies within an acute care hospital setting.
The ideal candidate will possess strong clinical assessment skills, acute care case management experience, and the ability to collaborate effectively with interdisciplinary healthcare teams to optimize patient outcomes and length of stay performance.
Location: Presque Isle, Maine
Position type: Minimum 6-month temp/contract role (possible extension)
Schedule: Fully On-Site | Monday - Friday | 8:00 AM - 4:30 PM
Pay Rate: $ 47.37/hour
Primary Responsibilities

Conduct comprehensive admission and ongoing case management assessments

Review and validate medical necessity and appropriate level of care
-
Monitor patient progression of care and support length-of-stay optimization
-
Collaborate daily with interdisciplinary teams to support discharge planning and prevent avoidable delays

Complete CMS-compliant reviews and documentation, including:

Admission reviews
-
Continued stay determinations
-
IMM and MOON administration
-
Coordinate care transitions and post-acute placements
-
Identify and resolve barriers to discharge in partnership with physicians, nursing staff, social workers, and ancillary departments

Maintain accurate and timely clinical documentation supporting medical necessity and regulatory requirements

Monitor high-risk patient populations and identify opportunities for early intervention

Support audit readiness and regulatory compliance initiatives

Adhere to all hospital policies, CMS Conditions of Participation, and organizational standards
Required Qualifications

Active Registered Nurse (RN) license required

Previous Case Management experience in an acute hospital setting

Strong patient-facing/bedside clinical experience

Experience using Cerner EHR required
