How you'll make an impact in this role
This is an exceptional opportunity for a motivated Nurse Practitioner to join an employed position within a busy, well-respected geriatric care facility, supported by a strong interdisciplinary team and the full resources of a robust health system network.

Join an established team of 1 physicians and 6 APPs

Call structure: Light 1:7 weekend call, very rare weekday calls, providing guidance to staff (not required to come in)

Practice setting: Longitudinal Primary Care/PACE Model, managing a smaller patient panel

Access to Strong interdisciplinary support team

Employed position within a nonprofit health system

Board Certified or Board Eligible as a Certified Nurse Practitioner (NP)

Eligible for state APRN licensure in Kansas and DEA registration

Provide comprehensive, value-based primary care and case management to frail elderly participants in the PACE program, focusing on quality outcomes

Collaborate closely with the interdisciplinary team (IDT) to ensure patient-centered decisions and holistic care

What minimum requirements you'll need
Licensure / Certification / Registration:

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

Nurse Practitioner obtained prior to hire date or job transfer date required.

Registered Nurse obtained prior to hire date or job transfer date required.
Education:
- Master's degree of Nursing required.
