We are seeking an experienced and compassionate Travel Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes throughout Cobb County. This role is ideal for an LPN with strong home health experience, excellent assessment skills, and a passion for helping patients manage chronic conditions and recover safely in the home setting.
The successful candidate will provide direct patient care, wound care management, disease process education, and ongoing assessments while collaborating with an interdisciplinary healthcare team to ensure high-quality patient outcomes.

Pay Rate: $36.18/hour (55.92/hour)
Schedule & Coverage:
Key Responsibilities

Provide skilled nursing care to patients in their homes according to physician orders and individualized care plans

Perform comprehensive patient assessments and report changes in condition to the supervising clinician

Deliver wound care treatments and monitor healing progress
-
Educate patients and caregivers regarding disease processes, medications, treatment plans, and self-care techniques

Monitor vital signs and evaluate patient response to treatment

Document patient visits, assessments, interventions, and outcomes accurately and timely

Collaborate with physicians, RNs, therapists, and other healthcare professionals to coordinate patient care
-
Reinforce patient safety practices and promote independence within the home environment
-
Ensure compliance with agency policies, home health regulations, and documentation standards
-
Maintain productivity and quality standards while managing a designated patient caseload
Required Qualifications

Active Licensed Practical Nurse (LPN) License
-
Previous Home Health experience required
-
Experience performing wound care and patient assessments

Working knowledge of Home Care Home Base (HCHB) software

Strong clinical documentation skills

Reliable transportation and willingness to travel throughout the assigned coverage area
