Job Description:
Schedule: Variable day shift times based on need.
Location: 601 E Rollins Street, Orlando, FL 32803
Educate and counsel patients, families, and healthcare team members on safe and effective medication use.
Complete Discharge Medication Reconciliations (DMRs)
-
Ensure medication history and admission medication reconciliation are entered appropriately
-
Resolve incorrect doses, outdated meds, and inappropriate therapy based on evidence-based guidelines by communicating with patients, caregivers, and providers
-
Reconcile inpatient orders with provider recommendations and patient's current health status to ensure discharge orders are appropriate
-
Work with inpatient case management and outpatient pharmacy services to ensure patient has access to medications upon discharge
-
Provide medication counseling including recommended drug monitoring and disease-specific goals when appropriate
-
Triage and resolve issues that arise post-discharge when appropriate
Participate in Quality Improvement Initiatives

Readmission reduction tracking

Medication discrepancy resolution and interventions

Cost-avoidance programs
