We are seeking an experienced Travel Registered Nurse (RN) for a Home Health/Hospice assignment serving patients throughout Louisville, Kentucky, and surrounding coverage areas. This position is responsible for providing comprehensive in-home nursing care, conducting OASIS assessments, and managing a diverse patient population with varying healthcare needs.
The ideal candidate will have strong home health experience, proficiency with OASIS documentation, and experience using Homecare Homebase (HCHB).
Location: Louisville, KY
Schedule: Monday-Wednesday, 8:00 AM - 4:30 PM
Employment Type: Temp for 1-year
Pay Rate: $ 55.92/hr.
Key Responsibilities

Conduct Start of Care (SOC), Resumption of Care (ROC), Recertification, and Agency Discharge visits.

Complete accurate and timely OASIS assessments and documentation.

Deliver skilled nursing care to home health and hospice patients in their homes.

Assess patient conditions and develop individualized plans of care.

Monitor patient progress and communicate changes to the interdisciplinary care team.

Educate patients and caregivers on disease management, medications, treatments, and preventive care.

Maintain compliance with agency, state, and federal regulations.

Document all patient interactions and clinical findings in Homecare Homebase (HCHB).
Required Qualifications

Active Kentucky Registered Nurse (RN) License.

Graduate of an accredited nursing program.

Minimum of one year of Home Health experience.

Minimum of one year of OASIS experience.

Experience using Homecare Homebase (HCHB).

Valid driver's license, reliable transportation, and auto insurance.

Ability to travel throughout the assigned coverage territory.
Preferred Qualifications

Previous travel nursing experience.

Strong assessment and critical thinking skills.

Excellent communication and patient education abilities.

Experience managing a high-volume home health caseload.
