ED Patient Case Manager RN (Onsite)
Shift/Hours:
-Full Time DAY and NIGHT shifts available
-Mix of 12 hour and 8 hour shifts available (FLEXIBLE)
Position Summary:
MercyOne Trinity is seeking a compassionate and detail-oriented Registered Nurse (RN) Care Coordinator to support patients across the care continuum. In this role, you will collaborate with interdisciplinary teams to develop, implement, and monitor individualized care plans that promote quality outcomes, patient safety, and efficient resource utilization.
Key Responsibilities:
Coordinate patient care plans across inpatient, outpatient, and post-acute settings
Collaborate with physicians, nurses, and community partners to ensure seamless transitions of care
Conduct patient assessments, identify needs, and develop care plans
Facilitate discharge planning and ensure appropriate follow-up services
Educate patients and families on treatment plans and available resources
Monitor patient progress and adjust care plans as needed
Support quality improvement, regulatory compliance, and documentation standards
Qualifications:
Current Registered Nurse (RN) license in Iowa
Bachelor of Science in Nursing (BSN) preferred
2+ years of clinical nursing experience (care coordination, case management, or acute care preferred)
Strong communication, critical thinking, and organizational skills
Experience with care management systems and interdisciplinary collaboration
