Job Summary and Qualifications
The Registered Nurse in Discharge coordinates and delivers high quality, patient-centered care by overseeing the effective management of patients into and out of the acute care hospital setting. The RN Discharge utilizes specialized assessment skills to ensure quality admission to the appropriate care areas, and also safe discharges of acute care patients requiring education and post hospitalization care.
· Obtains a nursing assessment history and ensures proper inpatient care unit admission. Takes appropriate additional action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect
· Teaches patients/families/caregivers about any non-medicinal follow-up measures, such as healthy diet and exercise, disease prevention, and/or other lifestyle changes. Prepares patients and families/caregivers for future self-management
· Completes additional discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge
Job Requirements
Experience
· 1+ years of clinical nursing experience as a Registered Nurse (RN)
Education
· Associate's degree in Nursing (ADN) or higher
Licenses and Certifications

Current State of Texas Registered Nurse (RN) license or Compact Registered Nursing License. If Compact License, 60 days to apply for Texas RN license; 6 months to obtain TX RN license

Basic Life Support (BLS) certification per St. David's Healthcare policy

Advanced Cardiovascular Life Support (ACLS) certification per St. David's Healthcare policy - Required within 6 months of hire

NIH Stroke Certification required within 30 days of hire
