Description:
The Clinical Liaison externally markets, generates referrals, clinically approves referrals, and secures new admissions for the Transitional
Care Unit (ST-SNF) and the Acute Rehab Unit (ARU). The liaison creates strategies and tactics to generate appropriate referrals into the
facility through external marketing to hospital discharge planners and physicians with a goal to always sustain 100%
occupancy
- REQUIRED: Registered Nurse licensed in the State of Pennsylvania
- 2+ years proven expertise in marketing Post Acute facilities to hospitals and physicians with strong aptitude to interpret clinical information
and process admissions rapidly; proven success in short- and long-term census growth. - Required: - Experience in this area with existing established relationships withing the area hospitals.
- 90% external marketing to area hospitals/physicians, with remaining 10% at community as required - Strive to attain and sustain 100% total
occupancy at all times within the Skilled Nursing Facility and Acute Rehab Unit.
- Create strategies and implement external marketing program with the goal of identifying and developing new referral feeder relationships with all possible referring professionals (community physicians/Hospitalists, discharge planners, social workers, case managers, nurses) at
area hospitals, rehabs, hospices, home care agencies, etc.
- Establish daily/weekly presence at top feeder hospitals to obtain referrals and approve/rapidly secure admissions on site; seize all
opportunities to promote community SNF bedside to family members/residents at hospital and convert appropriate discharges into admissions.

Interface daily with current SNF staff community physicians to capture new referrals real time and convert to admissions.

Clinically approve and convert all appropriate referrals into admissions to include rapid review and decision back to hospital per required
turnaround time; work in collaboration with facility NHA, Medical Director, DON, RNAC and/or Case Manager to review complex cases
- Drive rapid turnaround of ST-SNF and Acute Rehab referrals - Review daily census reports, arrange tours, process admissions through the
facility, interact with family members to execute admissions paperwork, log referrals onto shared spreadsheet, and attend daily morning
clinical meetings to provide updates on admissions via teams.

Periodically touch base throughout the day with hospital-based teams, providing status updates on potential admissions.

Obtain authorizations for patients' admission to Acute Rehab or ST SNF - Ensure that all required admissions contracts and supporting
documents with resident/POA/Physician are obtained and signed prior to admission.
- In collaboration with Leadership Team, Social Work and Case Management, arrange tours upon request from the family/resident
. - Possess thorough ongoing understanding of delivery of clinical services at the facility with an ability to accurately articulate care
needs/services to all internal and external clinical professionals and prospective patients.
