LPN - Columbia & Greene Counties - Full-Time (no weekends, no on-call, no mandatory overtime!)
Schedule: Monday-Friday 8a-4p
The Community Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hospice Home Care team. In this rewarding role, you will provide compassionate end-of-life care and support to patients and families in the comfort of their homes.

Position Responsibilities:
-
Provide skilled nursing care to hospice patients in the home setting.

Monitor patient status and communicate changes to the interdisciplinary team.

Support patients and families through education, symptom management, and compassionate care.

Assist with implementation of the hospice plan of care.

Collaborate with nurses, physicians, social workers, chaplains, home health aides, and other hospice team members to ensure quality care.
Qualifications Required:
-
Current New York State Licensed Practical Nurse (LPN) license.

1 Year LPN experience
-
1 or 2 years of homecare and/or Hospice experience preferred

Valid driver's license and reliable transportation.

Strong communication, documentation, and organizational skills.
No hospice experience? No problem. We welcome nurses who are passionate about caring for others and are committed to learning.

Pay Range: $24.00-$31.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
