Openings: 2 (1 Case Manager, 1 On-Call RN - 7 Days On/7 Days Off)

Position Summary
We are seeking experienced Travel Registered Nurses (RNs) to join a hospice care team providing compassionate, patient-centered care throughout the Bowling Green, Missouri service area. This opportunity includes openings for both a Hospice RN Case Manager and an On-Call RN. The ideal candidate will have prior hospice experience, strong clinical assessment skills, and experience using Homecare Homebase (HCHB) EMR.
Coverage Area

Seven-county service area

Hannibal and Northern Missouri territories

Travel throughout the assigned coverage area is required.

Expected to report to the office at least once per week.
Key Responsibilities
-
Provide comprehensive hospice nursing care to patients in their homes and care facilities.

Manage an assigned caseload of approximately 12-14 patients as a Hospice Case Manager.

Perform comprehensive patient assessments and develop individualized care plans.

Coordinate care with physicians, interdisciplinary team members, patients, and families.

Complete hospice admissions as scheduling allows.

Monitor patient condition and provide symptom management and end-of-life care.

Educate patients and caregivers regarding disease progression, medications, and hospice services.

Maintain timely and accurate documentation in Homecare Homebase (HCHB).

Participate in on-call and weekend rotation as assigned.
Productivity Expectations
Required Qualifications

Active Registered Nurse (RN) license.

Previous hospice nursing experience.

Experience using Homecare Homebase (HCHB) EMR.

Strong clinical assessment, care coordination, and communication skills.

Ability to work independently while managing a patient caseload.

Valid driver's license and reliable transportation.

Schedule: Day shifts with on-call and weekend responsibilities.

Mileage Reimbursement: Mileage is reimbursed between patient visits.

Travel: Required throughout the assigned seven-county service area.

Work Setting: Hospice/Home Health.

Dress Code: Professional scrubs (no specific color required).
Pay Details: $55.92 per hour
