Position Summary
We are seeking an experienced Travel Registered Nurse (RN) to provide compassionate, high-quality hospice care in Fayetteville, NC . This role is ideal for a skilled hospice nurse who is comfortable managing patient care independently, completing admissions, and coordinating care for patients and families. The ideal candidate will have hospice experience, PointCare EMR experience, and strong clinical assessment skills.

Up to 50 miles from the office location

Travel throughout the assigned service area is required
Key Responsibilities

Provide skilled hospice nursing care to patients in their homes and assigned care settings.

Manage an assigned patient caseload and coordinate individualized plans of care.

Complete hospice admissions and comprehensive patient assessments.

Monitor patient conditions and provide symptom management and end-of-life care.

Collaborate with physicians, hospice leadership, and interdisciplinary care teams.

Educate patients and families regarding hospice services, medications, and care plans.

Document patient care accurately and timely using PointCare EMR.

Communicate changes in patient status and coordinate appropriate interventions.

Participate in weekday and weekend on-call rotations as assigned.
Productivity Expectations
- Minimum productivity expectation of 25 points .
Required Qualifications
-
Active Registered Nurse (RN) license.

Previous hospice nursing experience.

Experience with PointCare EMR .

Current CPR certification.

Strong assessment, communication, and care coordination skills.

Ability to work independently while managing multiple patient needs.

Schedule
-
Monday-Friday: 8:00 AM-4:30 PM

On-call rotation:

Weekdays: 4:30 PM-8:00 AM

Saturday/Sunday: 8:00 AM-8:00 AM

Approximately 1-2 weekday and 1-2 weekend on-call shifts as assigned.

Pay Details: $55.92 per hour
