Position: Hospice RN
Pay Rate: $55/hr
Location: Salisbury, MD
Schedule: 8a-4:30p (M-F), 40 hours per week
Contract Length: 13 Weeks
Coverage Areas:
Key Responsibilities:

Deliver hospice patient care while maintaining accountability for clinical outcomes

Manage end-of-life care in patients' homes

Conduct initial evaluations within 48 hours of referral, initiate personalized plans, and collaborate daily with physicians and care teams

Provide critical symptom management, psychosocial support, and ensure meticulous clinical documentation
-
Exercise sound clinical judgment in an effective and efficient manner
-
Coordinate patient care with interdisciplinary team members and caregivers
-
Provide comfort-focused and end-of-life care to patients and families
-
Demonstrate critical thinking and problem-solving skills in patient management

Maintain compliance with established nursing standards and best practices
-
Assist other healthcare professionals in the management and coordination of patient care

Accurately complete documentation, including OASIS assessments when required
-
Support patients and families through education, advocacy, and emotional support
Assignment Details

Admissions and OASIS assessments

RN wound care visits as needed
-
Home-based hospice patient care throughout assigned coverage area

Documentation completed within OASIS charting system
Requirements:

Active RN License in the state of practice
-
Minimum 1 year of Home Health experience

OASIS experience preferred
