Job Title: Registered NurseJob Description
Join a dedicated team in implementing a behavioral health and health initiative. As a Registered Nurse, you will co-lead project integrated care teams, providing integrated care coordination for a panel of clients. You will ensure clients are linked to necessary community primary and specialty care, while monitoring chronic medical conditions and managing client data. Collaborate with local primary care providers and work with an evaluator and data team to measure client outcomes. Conduct health care screenings and assessments, and ensure client linkage to necessary care.
Responsibilities

Team with the Project Director in implementing a behavioral health and health initiative.

Co-lead integrated care teams composed of clinical leaders, psychiatry, case managers, and medical assistants.

Provide integrated care coordination to a patient panel of clients, ensuring linkage to community primary and specialty care.

Monitor clients with chronic medical conditions for further identification and linkage to appropriate health care services.

Ensure client data collection and EMR entry on blood pressure, body mass index, and lab results on a scheduled basis.

Collaborate with local primary care providers and provide patient information to manage patient care, with patient consent.

Conduct and review health care screenings and assessments.

Supervise and provide direction to case managers and medical assistants regarding client health issues.

Delegate assignments and monitor effectiveness to ensure best practices and compliance with operational plans and program scope.
Essential Skills
-
Bachelor's Degree in Nursing/RN license with 1-2 years of related experience in behavioral health or integrated care, or Licensed Practical Nurse (LPN) with 2-4 years of related experience including staff supervision.

Strong belief and commitment to integrated care models.

Knowledge, experience, and sensitivity to working with clients with behavioral health, addiction, and co-occurring disorders.

Ability to exercise good judgment and decision-making.

Ability to multitask with strong organization skills.

Knowledge and experience with evidence-based practices in behavioral health and primary care.

Commitment to using data to drive decision making and rapid cycle change in programming.

Proficiency in Microsoft Office Suite and Electronic Medical Record Software.

Excellent interpersonal and communication skills.

Ability to maintain a professional demeanor under pressure.

Ability to operate with a keen sense of urgency and commitment to quality.
Additional Skills & Qualifications
- Bilingual (Spanish-speaking) preferred.
Work Environment
Enjoy a team atmosphere working with mental and behavioral health patients. You will be overseeing a team of nurses, focusing on care coordination, patient advocacy, and improving long-term outcomes in a less traditional bedside nursing role. Work Monday through Friday, 8 AM to 5 PM, in a mission-driven environment aimed at expanding access to behavioral health services and whole-person care.

Pay and Benefits
The pay range for this position is $60000.00 - $90000.00/yr.
