Travel Pay: $20.01/hour
Travel Rate (based on 40 hours): Hourly $20.01, Stipend (Lodging $1,141.00 + M&I $490.00) = $1,631.00,
Total Comp :$2,431.40
Travel Eligibility: Candidates must live at least 50+ miles from the facility to qualify for Travel Pay.

Shift Details:
RN Case Manager Duties & Responsibilities:

The Hospice Nurse admits patients to service, assesses the patient s and family s physical, psychosocial, environmental, safety and developmental needs.

They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment.

This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed.
Minimum Qualifications
-
Current, valid, and unrestrictedCA license/certification in good standing, as required for the position.

Minimum of one (1) year Hospice experience as RN Case Manager

Registered Nurse license,

Valid BLS
-
Graduate of an Accredited School of Nursing

Additional certifications may be required

Copy of current, completedPrimary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
