Registered Nurse RN Case Manager Home Health
Deliver One-on-One Care Where Patients Feel Most Comfortable
Elara Caring is hiring a compassionate Registered Nurse Case Manager to serve home health patients throughout Wells, York, and surrounding southern Maine communities.
As a Home Health RN, you will manage an assigned caseload and provide personalized care that helps patients remain safe, independent, and comfortable at home. You will have the autonomy to organize your visits while receiving support from an experienced interdisciplinary team.

What You'll Do
-
Manage an assigned caseload of home health patients
-
Complete initial evaluations and ongoing nursing assessments
-
Develop, coordinate, implement, and update individualized Plans of Care
-
Monitor patients' physical, psychosocial, environmental, and safety needs
-
Provide skilled nursing treatments and patient education
-
Educate caregivers and family members on the patient's care needs
-
Communicate changes in condition to physicians, clinical leadership, and other care team members
-
Coordinate care with nurses, therapists, social workers, physicians, and community providers
-
Complete accurate and timely clinical documentation
-
Maintain patient privacy and confidentiality
-
Support safe transitions and continuity of care
What You'll Bring
-
Current and unrestricted Registered Nurse license in Maine
-
At least one year of clinical nursing experience
-
Strong assessment, communication, organization, and documentation skills
-
Ability to manage a caseload and work independently in patients' homes

Willingness to travel throughout Wells, York, and the assigned coverage area
-
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

Must be able and willing to travel 50%

Associate Degree or Bachelor's Degree in Nursing preferred

Previous home health or hospice experience preferred

HomeCare HomeBase experience preferred

Pay is based on education, experience, skills, and internal equity, with an anticipated salary range of $90,000 to $100,000 annually.
