Clinic - LPN
GhostRx is recruiting a full-time Licensed Practical Nurse (LPN/LVN) for the Ancillary Care Clinic at the Phoenix VA Health Care System's Carl T. Hayden Veterans' Affairs Medical Center, a tertiary care teaching hospital in central Phoenix with a 24-hour Emergency Department, a 24-bed ICU, and inpatient medicine, surgery, and psychiatry services supporting Veterans across Arizona. Working a Monday through Friday day tour (7:30AM - 4:00PM), you will support outpatient specialty and ancillary clinic operations by rooming Veterans, obtaining vital signs and health histories, performing medication reconciliation, administering injections and immunizations, assisting providers with procedures, and managing clinic flow so that Veterans are seen safely and on time.

This position offers an LPN a predictable weekday schedule with no nights or weekends in a busy, team-based outpatient setting within a federal teaching hospital. Qualification and Requirements

U.S. citizenship or lawful permanent resident (Green Card) status required to perform services under this federal contract

Ability to read, write, and speak English well enough to communicate effectively with patients, families, and the care team; basic computer literacy required

Active, current, and unrestricted Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in any U.S. state, territory, or commonwealth

Graduate of an accredited practical or vocational nursing program

Minimum of 1 year of LPN/LVN experience within the preceding 3 years, with outpatient clinic or ambulatory care experience preferred

Competency in vital signs, medication administration, injections and immunizations, specimen collection, and point-of-care testing

Strong customer service, organizational, and documentation skills

Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross

Must pass a test-based clinical competency assessment prior to submission and provide professional references with current contact information

Ability to successfully complete a federal background investigation, fingerprinting, and VA credentialing package, including primary-source license verification

Documented immunizations and TB screening; annual privacy, information security, and bloodborne pathogen training required

Willingness to complete orientation and unit training during day shift hours before transitioning to the assigned tour

Ability to work a Monday through Friday day tour, with occasional schedule adjustments as clinic needs require
Responsibilities
-
Room Veterans, obtain and document vital signs, chief complaint, and health history, and complete clinical reminders and screenings

Perform medication reconciliation and administer oral, intramuscular, and subcutaneous medications, injections, and immunizations as ordered

Assist physicians, nurse practitioners, and physician assistants with examinations and minor procedures, including set-up, patient positioning, and specimen handling

Perform point-of-care testing, EKGs, wound care, and dressing changes within the LPN scope of practice

Manage clinic flow, coordinate consults and follow-up appointments, and communicate with Veterans by phone and secure messaging

Maintain clinic supplies, equipment, and exam rooms in a clean, stocked, and ready condition

Provide patient education on medications, self-care, and follow-up instructions under RN and provider direction

Float to other clinics or units within your competency and skill set when directed by nursing leadership

Document care accurately and in real time in the VA electronic health record (CPRS/VistA) and communicate patient status changes during shift report

Comply with VA and Joint Commission standards, infection control and safety policies, HIPAA and the Privacy Act, and all facility rules and procedures

Complete required orientation, annual training, and competency validations and report to work as scheduled, including during emergencies or adverse weather
