Travel Rate (based on 40 hours): Hourly $35.43, Stipend (Lodging $1,274.00 + M&I $602.00) = $1,876.00,
Total Comp: $3,293.20
Travel Eligibility: Candidates must live at least 50+ miles from the facility to qualify for Travel Pay.

Shift Details:
Nursing Specialty: Patient Care Coordinator/ RN Case Manager
Duties & Responsibilities
-
Coordinate and manage patient care plans to support quality outcomes and continuity of care.

Assess patients' needs and develop individualized care plans based on clinical conditions and available resources.

Monitor patient progress and adjust care plans as needed.

Collaborate with physicians, nurses, healthcare providers, patients, and families to coordinate appropriate services.

Monitor utilization of healthcare services and help ensure appropriate and cost-effective care.

Maintain accurate and timely documentation of assessments, care plans, interventions, and patient outcomes.
-
Identify potential barriers to care and assist patients in accessing appropriate resources and services.
-
Communicate patient status and care needs effectively with the healthcare team.

Ensure patient care activities are performed in accordance with applicable policies, procedures, and professional standards.

Perform other related duties as assigned..
Minimum Qualifications:
-
Bachelors Degree in Nursing or Associate of Science in Nursing

Oregon & WA RN or compact license active and on file
-
Must have 1 year of case manager or PCC experience
-
Previous KP experience preferred
-
EPIC charting and experience with Medicaid/Medicare.

Additional certifications may be required

Copy of current, completedPrimary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
