Title: Transplant Nurse Coordinator
BayCare Health System is currently seeking a Transplant Nurse Coordinator to join our outstanding and compassionate team at St. Joseph's Hospital . As part of the community since 1934, St. Joseph's Hospital has been a partner for continued health and wellness.
The Transplant Nurse Coordinator responsibilities include:

Providing coordination through all phases of patient care.

Planning, directing, providing, and documenting patient care.

Assisting in coordinating comprehensive care for patients which includes assisting with the consent process, data collection and arranging any required follow-up visits, referrals, testing, treatment and long-term care follow-up.

Working with interdisciplinary team to coordinate on-going care.

Making referrals to external clinicians or treatment programs or community resources as appropriate.
