JOB DESCRIPTION
Licensed Practical Nurse (LPN) – Assisted Living & Home Care
Bethesda Sunrise Village
Position Summary:
Provides skilled nursing care to assisted living and home care residents under the Clinical Nurse Supervisor. Responsible for monitoring clinical conditions, administering medications and treatments, coordinating care, and supporting unlicensed staff to ensure safe, high-quality resident care.
Schedule:
64 hours per pay period with rotating on-call duties, including weekends and holidays. Shifts may vary with occasional short-notice coverage required.
Key Responsibilities:
- Assess, monitor, and report changes in resident condition to RN
- Commuting one day a week to Park View Village in Olivia, MN.
- Administer medications and treatments per physician orders and care plans
- Accurately document clinical care and maintain electronic health records
- Assist with admissions, transfers, and discharges
- Oversee and support unlicensed caregiver staff in daily duties
- Ensure compliance with state regulations, agency policies, and care standards
- Operate and maintain clinical equipment as needed
- Respond to emergencies and initiate appropriate protocols
- Collaborate with interdisciplinary team members, families, and providers
Qualifications:
- Current, unencumbered Minnesota LPN license required
- Graduate of an approved practical nursing program preferred
- Minimum 1 year of nursing experience (home care, long-term care, or acute care preferred)
- Strong clinical judgment, communication, and leadership skills
- Ability to work independently and as part of a care team
- Proficiency with electronic documentation systems
- Valid driver’s license required
- Must pass background check and TB screening
Physical Requirements:
- Ability to lift up to 50 lbs and assist with resident mobility
- Frequent standing, walking, bending, and physical activity
- Adequate vision, hearing, and manual dexterity for clinical tasks
Work Environment:
Assisted living and home care settings with exposure to clinical care needs, variable resident conditions, and moderate environmental demands.
