Position Summary
We are seeking an experienced Pediatric RN Case Manager to support care coordination and discharge planning for medically complex pediatric patients. The ideal candidate will have a strong background in pediatric case management and the ability to collaborate across interdisciplinary teams to ensure safe and effective transitions of care.
Responsibilities

Manage caseload of pediatric patients requiring complex care coordination

Lead discharge planning for medically complex pediatric cases

Collaborate with physicians, social workers, and interdisciplinary teams

Coordinate post-discharge services and resources
-
Ensure compliance with hospital policies and regulatory standards

Provide patient and family education related to care plans and transitions
Qualifications

Active Massachusetts RN license (required)

Bachelor's degree in Nursing (BSN) required

Minimum 3-5 years of nursing experience

Prior pediatric case management experience required

Demonstrated experience with discharge planning for medically complex pediatric patients

Case Management certification (CCM or equivalent) preferred

Flexibility to support department needs, including occasional weekend coverage
