The Nurse Care Manager provides professional, patient-centered nursing care in an ambulatory setting as part of a multidisciplinary care team. This role delivers both direct and telephonic nursing services, develops and maintains individualized care plans, and evaluates patient outcomes in accordance with established standards, policies, and procedures. The Nurse Care Manager documents and communicates care interventions, delegates appropriate tasks to licensed and unlicensed staff, and ensures compliance with regulatory standards while supporting patients across a range of chronic and acute conditions.
Responsibilities

Provide professional ambulatory nursing care that is patient-centered and aligned with the multidisciplinary plan of care and organizational policies and procedures.

Deliver direct and telephonic nursing services to patients and families, ensuring care meets standards for Ambulatory Care Nursing.

Collect and analyze patient health data in a continuous and systematic manner to identify needs, inform care planning, and support clinical decision-making.

Assist with the development, implementation, and ongoing maintenance of individualized care plans, revising outcomes and interventions based on ongoing evaluation.

Evaluate the effectiveness of nursing interventions and document outcomes in accordance with professional and organizational standards.

Facilitate individual or group nursing visits to address chronic illnesses and provide patient education on disease management and treatment.

Identify patients at risk for poor outcomes or those requiring more intensive services, and provide proactive outreach and frequent follow-up by phone and in person.

Provide complex case management, including chronic disease case management, critical diagnosis care coordination, transition care management, high-risk clinical tracking, complex medication management, and system utilization support.

Perform medication reconciliation and provide adherence counseling, assessing patient knowledge and identifying barriers to adherence and self-care.

Triages walk-in and same-day clinic patients to ensure timely and appropriate care.

Collaborate with the multidisciplinary team to address both physical and psychological aspects of health and provide comprehensive care for the whole patient.

Work closely with the Transition of Care team to support safe and effective transitions across care settings.

Collaborate with providers during appointments or nursing visits to deliver education on health promotion, disease prevention, and disease management.

Assist patients in developing self-management goals, evaluate progress toward these goals, and coordinate appropriate follow-up with the care team.

Access and coordinate appropriate internal and external resources to meet patient and caller needs.

Demonstrate awareness of cultural, ethnic, gender, sexual orientation, and racial differences and their potential impact on patient outcomes, and incorporate this awareness into care.

Teach patients self-care skills, including medication administration (such as self-injections, nebulizers, and inhalers), wound care, and monitoring of blood sugars and blood pressures.

Provide education on the importance of preventive health screenings and encourage adherence to recommended preventive care.

Delegate clerical and non-nursing clinical tasks to assistive personnel appropriately and in accordance with scope of practice.

Utilize interpreter services when needed to ensure clear and effective patient communication.

Attend nursing departmental meetings, site staff meetings, and committee meetings as assigned to support quality improvement and coordination of care.

Document incoming and outgoing calls in real time and communicate effectively with team members to ensure continuity of care.

Document telecommunications that accurately reflect care provided and address actual or potential health needs of callers and patients.

Complete reviews of systems and physical assessments, correlating findings with patient health histories to prepare patients for further examination or treatment.

Ensure compliance with regulatory standards, including those of relevant accrediting bodies and patient-centered medical home models.

Delegate tasks to other licensed and unlicensed care providers appropriately while maintaining accountability for patient outcomes.

Demonstrate flexibility in scheduling to meet the demands of the clinic schedule and patient needs.
Essential Skills

Current Registered Nurse (RN) licensure in the state of Maryland.

Graduate of an accredited school of nursing.

Experience documenting in an Electronic Medical Record (EMR).

Ability to provide ambulatory nursing care, including assessment, care planning, implementation, and evaluation.

Strong analytical and critical thinking skills to evaluate patient needs, identify at-risk patients, and modify care plans.

Competence in complex case management, including chronic disease management, care coordination, transition care management, and medication management.

Ability to perform medication reconciliation and provide adherence counseling.

Skill in triaging walk-in and same-day patients in an ambulatory setting.

Effective collaboration skills to work within a multidisciplinary team and transition of care team.

Strong patient education skills, including teaching self-care techniques, disease management, and preventive health practices.

Ability to delegate tasks appropriately to licensed and unlicensed personnel while maintaining accountability.

Proficient communication skills, both verbal and written, including clear documentation of patient encounters and telecommunications.

Additional Skills & Qualifications

Preferred: One year of nursing experience in an ambulatory or acute care setting.

Preferred: Bachelor of Science in Nursing (BSN).

Significant related experience with the populations served may substitute for preferred experience requirements.

Experience in case management and managed care environments.

Background or experience in OB/GYN, labor and delivery, or women's health.

Experience in acute care, utilization management, or healthcare navigation.

Experience providing patient education and coordinating referrals.

Familiarity with electronic health records and healthcare documentation standards.

Ability to work with computers, including word processing and database applications.

Experience working with uninsured and low-income populations and a commitment to serving vulnerable communities.
Work Environment
This role is based in an ambulatory care center located in the heart of the Mt. Vernon area of Baltimore, within a historic downtown building that houses Adult Primary Care, OB/GYN, and additional specialties. The Nurse Care Manager works in a multidisciplinary clinical setting with exposure to multiple specialties and a broad range of patient needs, including uninsured and low-income populations. The schedule follows standard Monday-Friday daytime hours with no weekend shifts, supporting a stable work-life balance. The environment is team-oriented and mission-driven, emphasizing comprehensive, full-service care and collaboration across disciplines. Clinical work is supported by electronic medical record systems and standard office and clinical equipment. The role operates in a professional healthcare setting where appropriate clinical attire is expected.

Pay and Benefits
The pay range for this position is $40.00 - $45.00/hr.
