Adecco Healthcare & Life Sciences is currently hiring a Travel Registered Nurse (RN) - Home Health in Clearfield & Centre County, PA.
This is an on-site, full-time position with daily travel to patient homes.

Type: Contract (04/24/2026 - 09/30/2026) - possibility of extension
Schedule: Monday - Friday (8:00 AM - 4:30 PM)

Primary Responsibilities:

Provide skilled nursing care in the home health setting under physician orders and in compliance with applicable laws, regulations, and agency policies

Perform OASIS assessments, start of care visits, return visits, and ongoing patient evaluations

Develop, implement, and revise individualized care plans based on patient needs and clinical findings

Deliver high-quality clinical care including IV therapy, wound care, and wound VAC management
-
Conduct venipuncture and administer treatments as required
-
Provide patient and caregiver education on disease management, medications, and home care techniques
-
Ensure accurate, timely, and compliant documentation in accordance with agency and payer requirements

Coordinate care with physicians and interdisciplinary healthcare team members to ensure continuity and quality outcomes

Monitor patient conditions and identify changes requiring intervention

Travel daily to patient homes across assigned coverage areas and provide care in multiple locations within each shift

Participate in on-call and weekend rotation as required

Ensure compliance with all home health policies, procedures, and safety standards
Qualifications:
-
Active Registered Nurse (RN) license in the state of Pennsylvania required

Current BLS certification preferred

Minimum home health experience required

Experience with OASIS documentation required

Hands-on experience with IV therapy, wound care, wound VACs, venipuncture, and patient disease education required

Strong clinical assessment, documentation, and care coordination skills required
