Medical Social Worker MSW (PRN)

What You Will Do

Complete social work assessments in the home
-
Identify psychosocial, financial, and environmental needs

Support care planning and discharge coordination

Connect patients and families to community resources
-
Provide counseling and emotional support

Collaborate with nurses, physicians, and care teams

Document visits and updates in a timely manner
What You Need
-
Master's Degree in Social Work (MSW)
-
Active state social work license
-
1+ year healthcare or home health experience preferred
-
Strong communication and organization skills
-
Reliable transportation and willingness to travel (50%)
-
Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
