SUMMARY
The Registered Nurse works as part of a Forensic Assertive Community Team that provides care and treatment for clients that are experiencing significant mental health/psychiatric symptoms, who are demonstrating significant functional impairments, and who may have co-occurring chemical dependency and/or personality disorder. Primary duties include health/medication assessment, crisis assessment and management, planning, individual counseling, skills teaching, psychiatric/medical case management, medication monitoring/administration, staff training, care coordination with outside providers, and on-call.*FACT is an adaptation of the assertive community treatment model designed to serve people with a serious mental illness who are involved in the criminal justice system.
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.

Work in full partnership with the client in his/her recovery process.

Promote for the care, welfare, treatment, and safety of the clients as individuals who may have significant mental health/psychiatric symptoms and may have co-occurring chemical dependency and/or personality disorders.

Conduct health/medication assessment, crisis assessment and management, planning, individual counseling, skills teaching, psychiatric/medical case management, medication monitoring/administration, staff training, care coordination with outside providers, and on-call.

Provide crisis and behavioral management techniques, goal directed counseling, and medical/psychiatric case management.

Work with a forensic assertive community team with an adaptation of the assertive community treatment model designed to serve people with a serious mental illness who are involved in the criminal justice system. Work collaboratively with the mental health professionals and the criminal justice agencies.

Deliver nursing care, monitors treatment, and administers medication to individuals with varying levels of mental health disorders and/or intellectual and developmental disabilities, in accordance with established nursing practices and guidelines.

Coordinate communications and actions of various disciplines as it relates to health and safety support for the patient.

Complete daily, monthly, quarterly and annual nursing documents as/ if required and ensure documentation of individual's health is in nurse's notes.
SUPERVISORY RESPONSIBILITIES
Minimum Knowledge and Skills required for the Job
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job.
Education and Experience:
- Associate arts degree in nursing and has at least 6,000 hours of supervised experience in the delivery of services to persons with mental illness; or bachelor's degree in nursing and has at least 2,000 hours of supervised experience in the delivery of services to persons with mental illness; or holds a graduate degree in nursing and has at least 2,000 post BA hours of supervised experience in the delivery of services to persons with mental illness.
Certificates, Licenses, and Registrations:
- All staff are supported in gaining certification as a Certified Psychiatric Rehabilitation Practitioner.

- Medium Work: Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
