Per Diem LVN (Residential Home Setting) | $40 per hour | Santa Rosa, CA
Flexible AM / PM / NOC Shifts | Pick Up Extra Hours
We're seeking dependable Licensed Vocational Nurses (LVNs) for a per diem opportunity in Santa Rosa, CA . This role is ideal for clinicians looking to pick up additional shifts and earn extra income with flexible scheduling. Shifts are available in advance and also to help cover call-offs.
Position Highlights
Location: Santa Rosa, CA
Setting: Residential Group Homes
Schedule: Per Diem (as needed)
Shifts Available:

AM: 7:00 AM - 3:30 PM
-
PM: 3:00 PM - 11:30 PM
-
NOC: 11:00 PM - 7:30 AM
Role Focus
-
Perform general nursing care and procedures within scope of practice
-
Provide supervision, behavior support, and direct resident care
-
Participate in interdisciplinary care planning and support plans
-
Oversee DSPs/CNAs when functioning as lead staff
-
Deliver active programming and structured support throughout shifts
-
Maintain accurate documentation and follow care protocols
-
Report concerns in accordance with mandated reporter guidelines
Required Qualifications
-
Active CA LVN or LPT license (Required)
-
High School Diploma or GED
-
Valid CA driver's license with acceptable driving record
-
Minimum 1 year of experience in healthcare or human services preferred

Strong communication, leadership, and clinical skills

Reliable and flexible to pick up shifts
Additional Requirements

Must meet all federal, state, and facility requirements

Understanding of HIPAA and patient/resident confidentiality

Must be at least 18 years of age
