Drive improved patient outcomes by bridging hospital and outpatient care, ensuring seamless continuity and reducing care fragmentation.

Reduce hospital readmission rates by providing ongoing clinical monitoring and early intervention for post-discharge patients.

Empower patients through personalized health coaching, education, and support for long-term treatment goals.

Coordinate care plans among primary care providers, specialists, and community resources, addressing social determinants of health and medication management.

Transform patients into active participants in their recovery, resulting in measurable improvements in quality metrics for the Total Quality Management department.
What minimum requirements you'll need
Licensure / Certification / Registration:

Licensed Registered Nurse obtained prior to hire date or job transfer date required.

Registered Nurse credentialed from the Oklahoma Board of Nursing required.

BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
What additional preferences we're seeking

Must be willing to obtain a compact license within 90 days of hire

Prior Care Manager, Case Manager in Home Health/Hospice, or Patient Navigator in Ambulatory setting

Willing to travel within 40 mile radius of office for in-field component conducting patient home visits and attending specialty appointments (30%)
