Job Title: Registered Nurse Supervisor - Care Transitions Clinical Specialist
Job Description
The Care Transitions Clinical Specialist provides support and leadership to Recuperative Care nursing and support staff. This role involves utilizing reflective and proactive staff development techniques to build and model a strengths-based, client-centered culture. The specialist coordinates the administration of medical services to clients, including assessing, screening, educating, and treating patient conditions. Additionally, this role oversees medication management workflows, practices, policies, monitoring, and implementation of patient self-administration of medications to minimize risk.
Responsibilities

Successfully complete the credentialing process if applicable.

Work as part of a multidisciplinary team with patients experiencing homelessness and co-occurring disorders such as mental health, chronic medical conditions, and substance use disorders at a high acuity care facility requiring 24/7 patient support and monitoring.

Train nursing, support staff, and new hires in nursing and patient support duties, medication management, policies, and processes.

Oversee nursing and support staff functions and compliance with established guidelines, protocol, and best practices.

Perform RN duties for patients receiving medical respite (recuperative care), including preparing rooms, rooming patients, taking medical histories, recording vital signs, taking temperatures, completing assessments, and performing venipuncture and immunizations as authorized and needed.

Provide education and oversight to ensure best practices in client care and adherence to certifying and regulatory standards are met by all nursing and support staff.
Essential Skills

CA RN License

2+ years of supervisory experience

Proficiency in charts, EMR systems, patient coordination, and medical terminology

Work Environment
The work environment includes supervising up to 15 individuals. The role offers flexible shifts, Monday to Friday from 11am to 7pm, with the opportunity to support PM staff starting in the afternoon. Occasionally, there may be a requirement to start at 6:00am to support the night shift. Benefits include medical, dental, and vacation. Pay and Benefits
The pay range for this position is $122000.00 - $150000.00/yr.
