Responsibilities:

The Faith Community Nursing (FCN) Coordinator supports the development, implementation, and evaluation of faith community nursing programs in assigned communities.

Serving as a liaison between the healthcare system, local faith communities, and the community at large.

The coordinator guides both paid and volunteer FCNs in providing holistic care that integrates physical, emotional, and spiritual well-being.

Key responsibilities include recruiting and orienting FCNs, assessing health needs, coordinating and performing wellness screenings and education events, as well as presentations to nurses and clergy.

The coordinator also maintains program data, ensures compliance of data collection from FCN Volunteer and self reporting on Community Benefit Form.

Fosters partnerships with congregations and community organizations to align services with BayCare's health priorities and Community Health Needs Assessments (CHNAs).

The faith community nurse coordinator is responsible to coordinate care of the clients enrolled in the FCN Transitional Care Program.

Responsible for transportation between facilities and to community events.

Other duties as assigned.
Minimum Qualifications:
Required Education: Associate's degree in Nursing or Diploma Nursing. Preferred Education: BSN (Bachelor of Science in Nursing).
Required Certification: BLS (Basic Life Support) and RN (Registered Nurse).
Required Experience: Minimum 3 years of direct patient care experience. Working knowledge and computer proficiency (Microsoft suite).
