We are seeking an experienced and patient-focused Nurse Practitioner (NP) to provide comprehensive primary care services within a Family/Internal Medicine clinic. The Nurse Practitioner will work collaboratively with physicians and medical assistants, and support staff to deliver high-quality care to patients across all age groups.
This role offers the opportunity to manage a diverse patient population while practicing independently within a supportive clinical environment utilizing Epic EMR.
Location: Rialto, California
Position type: Minimum 6-month temp/contract role (possible extension)
Schedule: Fully On-Site | Monday - Friday | 8:00 AM - 4:30 PM
Pay Rate: $ 72.37/hour
Key Responsibilities

Perform comprehensive patient examinations and health assessments

Obtain and review detailed medical histories

Diagnose and manage acute and chronic medical conditions

Order, perform, and interpret diagnostic and laboratory tests

Prescribe medications and treatment plans in accordance with state regulations and clinical guidelines

Conduct Annual Wellness Visits and preventive health screenings

Complete post-hospital discharge follow-up visits and transitional care management

Manage patient communications and care coordination through Epic In-Basket messaging

Provide patient education regarding diagnoses, treatment plans, medications, and preventive care

Collaborate with physicians, medical assistants, and interdisciplinary team members to ensure continuity of care

Maintain accurate and timely clinical documentation in Epic EMR

Adhere to all organizational, regulatory, and clinical quality standards

Required Qualifications

Graduate of an accredited Nurse Practitioner program

Current California Nurse Practitioner License

Board Eligible or Board Certified Nurse Practitioner

Current BLS/CPR Certification

Active DEA Registration

Ability to independently evaluate, diagnose, and manage patients in an outpatient setting
Preferred Qualifications

Minimum of one year of outpatient clinic experience

Family Practice or Internal Medicine experience

Experience utilizing Epic EMR

Strong communication and patient education skills

Ability to work efficiently in a fast-paced clinical environment
