This is an on-site, full-time contract opportunity supporting home health patient care in the Moline and surrounding areas. Apply with a current resume for consideration.
Pay: $54 to $56/hr

Contract Dates: 06/22/2026 to 09/25/2026
Schedule: Monday - Friday | 8:00 AM - 4:30 PM
Weekly Guarantee: 32 Hours
Location: Moline, IL 61265
Mileage Reimbursement: Yes (between visits, from first patient to last patient)
Primary Responsibilities:
· Provide direct skilled nursing care as a Field RN in home health settings
· Manage and coordinate patient care plans in collaboration with clinical teams
· Perform patient assessments, documentation, and care interventions
· Support medication management and patient education as needed
· Ensure accurate and timely OASIS and clinical documentation compliance (if applicable)
· Communicate effectively with physicians, patients, and interdisciplinary staff
· Follow all infection control, privacy, and safety protocols
· Travel throughout assigned service areas including Moline, IL and surrounding communities
· Participate in on-call rotation as scheduled
· Perform additional duties as assigned
Qualifications:
· Active RN license required
· Experience in home health (HH) required
· Experience with HCHB required
· Strong clinical assessment and patient care coordination skills
· Ability to work independently in a field-based role
· Reliable transportation and valid driver's license required
