Full-Time | Monday - Friday | 8:00 AM - 5:00 PM

What You'll Do
As a Patient Care Manager, you will:
Lead and supervise clinical staff to deliver top-quality patient care
Coordinate and oversee individualized patient care plans
Ensure compliance with Medicare & regulatory standards
Collaborate with patients, caregivers, and healthcare providers
Drive performance through quality improvement initiatives
Mentor and develop staff through coaching and education
Promote a culture of excellence, safety, and compassion
What You Bring

Active Registered Nurse (RN) license
-
Home Health experience
-
2+ years of leadership/supervisory experience preferred

Knowledge of Medicare guidelines & regulatory standards

Strong leadership, communication, and organizational skills

Pay Details: $100,000.00 to $110,000.00 per year
