We are seeking an experienced Travel Hospice Registered Nurse (RN) to join a dedicated hospice team serving patients throughout the Clarksville, TN area. This role requires a compassionate clinician who is comfortable working independently, managing a caseload, and providing high-quality end-of-life care in the community.

Location: Clarksville, TN
Assignment Dates: Minimum 3-month temp/contract position (possible extension)
Employment Type: Travel Contract
Schedule: Monday-Friday, 8:00 AM - 4:30 PM, with on-call responsibilities
Hourly Pay Rate: $ 55.92/hr.
Responsibilities

Conduct comprehensive patient assessments and manage ongoing hospice care.

Perform medication reconciliation and coordinate prescription refills.

Complete all required clinical documentation and charting in a timely manner.

Document weekly Interdisciplinary Group (IDG) notes and participate in weekly IDG meetings.

Communicate regularly with physicians regarding patient condition, care plans, and clinical needs.

Coordinate care with hospice staff, caregivers, and other healthcare providers.

Respond to on-call needs as part of the clinical team rotation.

Maintain compliance with hospice regulations, agency policies, and quality standards.
Caseload & Coverage Area

Average of 7-8 patient visits per day (may vary based on census and patient needs).

Service area includes six counties surrounding Clarksville, TN.

Nurses are generally assigned a designated territory; however, coverage areas may change based on census and operational needs.
Qualifications
Required:
-
Active, unrestricted Tennessee RN license or Compact RN license.

Recent Hospice Nursing experience.

Experience using Homecare Homebase (HCHB) charting system.

Ability to work independently and collaboratively with physicians and interdisciplinary team members.

Strong assessment, prioritization, and time management skills.

Ability to participate in an on-call rotation.
Additional Information
- Mileage reimbursement is provided between patient visits; mileage from home to the first patient visit and from the last visit back home is not reimbursed.
