This is a full-time opportunity for a Patient Care Manager (Registered Nurse) supporting home health operations in Brooklyn Park, MN.
Salary: $90,000 - $110,000/year depending on experience
Schedule: Monday-Friday | 8:00 AM - 5:00 PM

The Opportunity
Join a collaborative clinical leadership team focused on delivering safe, effective, and patient-centered home health care. The Patient Care Manager plays a key role in overseeing clinical operations, supporting field staff, coordinating patient care, and ensuring services meet quality, compliance, and regulatory standards.
This position is ideal for an experienced Registered Nurse who enjoys leading teams, improving patient outcomes, and supporting clinicians through coaching, collaboration, and strong clinical oversight.
Key Responsibilities

Provide clinical supervision and leadership to ensure patient care aligns with professional standards, organizational policies, and regulatory requirements

Coordinate, monitor, and support individualized patient care plans

Oversee clinical personnel to ensure safe, effective, and consistent delivery of services

Collaborate with patients, caregivers, physicians, referral sources, and internal teams to support continuity of care

Review care delivery processes and support quality improvement initiatives

Ensure compliance with Medicare Conditions of Participation and applicable healthcare regulations

Assist with audits, documentation reviews, reporting requirements, and compliance initiatives

Promote a culture of safety, accountability, teamwork, and service excellence

Coach and mentor clinical staff through ongoing support and professional development

Help maintain efficient office and clinical operations while prioritizing patient needs
Qualifications

Active Registered Nurse (RN) license in the applicable state

Previous home health experience preferred

Minimum of 2 years of management, supervisory, or clinical leadership experience preferred

Knowledge of Medicare Conditions of Participation, home health regulations, and clinical standards

Strong communication, organization, and leadership skills

Ability to manage multiple priorities in a fast-paced healthcare environment

Pay Details: $43.00 to $53.00 per hour
