Duties Description
The Division of Residential Support creates and implements policies, programs, services and regulations to meet the health and long-term care needs of all New Yorkers and promote aging in place with dignity and independence. Within the Division of Residential Support, the Bureau of Intake Management receives, triages and assigns written and/or verbal complaints, and facility reported incidents, received by the Department concerning adult care facility and assisted living programs, nursing home, and licensed home care providers. Complaints and facility reported incidents are triaged on intake and assigned a level of severity that determines the timeframe in which the complaint is investigated. All relevant information provided is entered into a centralized complaint and incident tracking system and, if necessary, assigned to one of the Department's four regional offices for investigation. The results and findings of each investigation are then documented in a secure database.
The Bureau of Intake Management is staffed from 8:45 AM to 4:45 PM Monday through Friday, excluding holidays. Some weekend and holiday coverage is required. Complaints and facility reported incidents received after hours or on holidays that have been determined to be of an emergency nature are immediately referred to the regional office contact person for prompt follow-up.
Minimum Qualifications : Candidate must be a Registered Professional Nurse, licensed and currently registered to practice nursing in New York State or possess a limited permit to practice nursing in New York State. In addition, the Candidate must have excellent telephone, listening and written communication skills; working knowledge of all Microsoft Office products and experience with complex data entry systems.
Preferred Qualifications: Must have strong interpersonal, problem solving, decision-making and time management skills and the ability to communicate effectively with all internal and external stakeholders. In addition, the ideal Candidate must have excellent writing and editing skills and be able to effectively interpret laws and regulations governing adult care and assisted living facilities in New York State.
Responsibilities : This incumbent will be responsible for processing Metropolitan Area Regional Office facility reported incidents. This includes triaging and referring them appropriately. Functions include:
· Review facility reported incidents to determine triage decision and finalize initial report
· Contact facilities to obtain additional information if necessary;
· Prepare and send correspondence to facilities, as appropriate.
· Respond to general inquiries regarding the intake process;
· Participate in process improvement activities including daily case review;
· Assist with Management Reports;
· Participate in training and continuing education activities.
Weekend/Holiday Requirements:
Staff is expected, on select Saturdays and Sundays/holidays, to perform the following activities:
· Retrieve voice-mail messages;
· Triage and record relevant information retrieved during weekend or holiday hours.
· Refer emergencies per established policies and procedures.
This is part of a rotating schedule with other Bureau staff.
