Ghost RX is hiring five (5) Licensed Practical Nurses (LPNs) for a 52-week contract assignment at the Franklin Delano Roosevelt (FDR) Campus of the VA Hudson Valley Health Care System in Montrose, New York. The FDR Campus is home to extensive long-term care and mental health programs, including the Community Living Center (CLC), inpatient mental health units, a nationally recognized PTSD residential rehabilitation program, and substance abuse and homeless veteran services. In this role, you will provide individualized practical nursing care to veteran residents within the CLC or one of the campus's mental health units - administering medications, monitoring vital signs, assisting with activities of daily living, supporting wound and skin care, collaborating with the interdisciplinary team, and documenting care accurately in the VA electronic health record (CPRS). Candidates with prior experience in either Community Living Center / long-term care nursing OR VA-style inpatient mental health will be strongly considered. Shifts are flexible - 8-hour or 12-hour options are available - at 40 hours per week.

Qualification and Requirements
-
Experience in a Community Living Center (CLC), long-term care, skilled nursing, or VA/inpatient mental health setting is required

Active, unrestricted Licensed Practical Nurse (LPN) license in New York State (or compact-equivalent eligibility)

Graduate of a state-accredited Practical Nursing program approved by the state board of nursing

Current Basic Life Support (BLS) certification through the American Heart Association

Demonstrated ability to administer medications safely (including oral, IM, SC, and topical) and document accurately

Working knowledge of geriatric care, dementia care, hospice/palliative care, and behavioral health interventions

Familiarity with VA standards of care, person-centered care principles, and the HATCH model preferred

Prior VA, IHS, DoD, or other federal healthcare experience strongly preferred

Proficiency with electronic health records; VA CPRS/VistA familiarity is a plus

Ability to work 8-hour or 12-hour shifts, including weekends and holidays as required by the unit schedule

Strong observation, communication, and documentation skills

Ability to successfully pass federal credentialing, fingerprinting, and background investigation

Responsibilities
-
Provide safe, individualized practical nursing care to veteran residents in the Community Living Center (CLC) or mental health unit, spanning long-term care, short-stay skilled care, rehabilitation, hospice and palliative care, and acute behavioral health

Administer medications (oral, IM, SC, topical, and as ordered) accurately and on schedule, including psychotropic medications when assigned to mental health units

Monitor and document vital signs, intake and output, blood glucose, weights, and changes in resident condition
-
Assist residents with activities of daily living (ADLs) including bathing, dressing, toileting, mobility, and feeding while preserving dignity and independence
-
Perform wound care, dressing changes, catheter care, ostomy care, and other treatments under the direction of the registered nurse or provider
-
Recognize and respond to changes in resident condition, including transitory mental status changes, behavioral disturbances, and acute psychiatric symptoms; escalate to the RN appropriately

Collaborate with the interdisciplinary team (RNs, physicians, social workers, therapists, dietitians, chaplains, and recreation therapists) in developing and carrying out the individualized plan of care

Provide education to residents and family members regarding disease processes, medications, and prescribed treatment regimens

Document all care, observations, and resident responses accurately and timely in the VA electronic health record (CPRS)

Participate in resident-centered care planning, supporting choice, privacy, and quality of life in the CLC home environment

Maintain compliance with VA policies, Joint Commission standards, CMS long-term care regulations, HIPAA, and applicable federal regulations

Support a safe environment through infection prevention, fall reduction, medication safety, and emergency response readiness
