Medical Practice LPN position in the Primary Care Geriatric Medical Program

Summary:
To assist the clinician in providing medical care to the patients of the Medical Practice.
Responsibilities:

Prepares materials, supplies, equipment, and the patient to assist clinician with the patient's medical exam.

Obtains the patient's history and medical information.

Provides direct patient care as directed by a Registered Nurse or clinician.

Collects and processes specimens as directed by the clinician and performs point of care testing.

Administers medications, and vaccinations as directed by the clinician. Instructs the patient and family on how to perform clinician recommended preventive care, takes prescribed medication, prepares for tests, monitors treatment progress and notes symptom changes.

Contacts the patient to provide requested follow-up, advises them of test results or relays clinicians instructions.

Fulfills and adheres to all compliance responsibilities related to the position.

Performs other duties as required.
Education Skills Experience:
Ability to travel between practices as needed. Required: Active LPN License based on state; BLS certification required within 30 days.

Physical Effort: Sedentary/light effort. May exert up to 10 lbs. force
