Seeking a Travel RN for a home health setting responsible for providing comprehensive in-home patient care, care coordination, and skilled nursing services. This role requires strong independence, clinical judgment, and proficiency in home health documentation and regulatory compliance.
Location: Owensboro, KY
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Employment Type: Temp for a minimum of 3-months (likely extension)
Pay Rate: $ 55.92/hr.
Key Responsibilities

Conduct comprehensive patient assessments in home settings
-
Develop, implement, and manage individualized plans of care
-
Perform skilled nursing interventions including:
-
Wound care and wound vac management
-
Catheter care
-
Medication administration and management
-
Injections and IV antibiotic therapy
-
PICC line care and maintenance
-
Patient and caregiver education
-
Disease management and care coordination
-
Ensure accurate and timely clinical documentation
-
Maintain patient safety and risk reduction practices
-
Ensure compliance with regulatory and Medicare home health standards
Required Qualifications
-
Active RN license
-
Current CPR certification
-
Valid driver's license
-
Ability to work independently in patient homes

Strong communication (verbal and written)
-
Basic computer skills and EMR proficiency
Preferred / Nice to Have

Prior home health experience
-
Experience with Homecare Homebase (HCHB)

Proficiency with OASIS assessments (SOC, ROC, recertification, transfers, discharge)

Knowledge of Medicare home health regulations

Strong documentation and scheduling coordination skills

Productivity expectation: 30 points/week minimum
